The challenge

The problem we set out to solve

Patient engagement between visits is one of the hardest problems in healthcare: adherence to care plans slips, follow-ups get missed, and small issues escalate into avoidable admissions. Care teams are stretched thin, spending countless hours on manual outreach — calling to confirm medications, chasing check-ins and answering repetitive questions — work that scales linearly with the patient panel and never with staff capacity. The result is a coverage gap, where patients who need a nudge or a quick answer at 9pm on a Sunday simply do not get one. Closing that gap by hiring is prohibitively expensive, and the sensitive nature of health data means any automated solution has to clear a high bar for privacy and clinical safety before it can touch a patient.

How we built it

Design HIPAA-grade data handling first

We architected the platform around protected health information from the outset, with encryption, access controls and auditability appropriate for healthcare workloads on AWS. Privacy and safety were treated as foundational constraints, not features bolted on later.

Build intelligent, empathetic messaging

We used OpenAI to power conversational messaging that understands patient replies and responds with clear, on-plan guidance. The assistant handles routine questions and reminders in natural language rather than rigid scripts.

Automate structured health check-ins

We built automated check-in flows that proactively reach patients to confirm adherence, capture symptoms and surface concerns. This turns passive monitoring into consistent, low-effort touchpoints across the whole panel.

Route clinical signals to a human-in-the-loop

We implemented escalation logic so that any response indicating clinical risk is routed to a care-team member rather than handled autonomously. The AI extends the team's reach while keeping clinicians in control of clinical decisions.

Deliver on the channels patients already use

We integrated Twilio and a React Native client so outreach meets patients on messaging and mobile without forcing them into a new app or portal. Meeting patients where they are is what makes engagement stick.
